When lava buckets are used to smelt with a Dwarven Forge, buckets are duplicated. The buckets also stop the forge from grabbing new fuel from the input chest.
Probably arose because 1.3 fixed the issue where buckets disappeared from furnaces.
Could also be DF not playing nice with the bucket fuel. I personally can't say because I always have used coal or charcoal but let it be known: this is an exploit and duplication is a form of cheating. This applies to everyone.
